You're a strong patient advocate with a service-first approach, solid case management skills, and experience working in mental health, social or human services, or community relations. You bring strong interpersonal skills and the ability to build effective relationships with patients, internal teams, and external providers and community members. What You'll Bring to the Table

Education and required experience:
  • 3+ years of experience:
    • Working in a mental health treatment center, social work setting, community relations, or human services environment
    • In discharge planning
    • In EMR documentation
  • Working knowledge of diagnostic assessments, addiction and related interventions
  • High level of emotional intelligence 
  • Strong analytical and problem-solving skills 
  • A high school diploma is required; a bachelor’s degree is preferred
  • Ability to drive locally up to 30%
In addition, the following core competencies and characteristics are desired:
  • Strong interpersonal and communication skills to build rapport with clients and stakeholders, with strong customer service relations and performance improvement skills
  • Excellent organizational and time management abilities to manage multiple cases effectively
  • Understanding of mental health disorders and treatment options to provide informed support   
  • Familiarity with community resources and support services available for clients
  • Basic computer skills, including word processing, spreadsheets, calendars, and presentations
You will work onsite, Monday through Friday. This hourly role pays $25 to $29 per hour, depending on your experience. Note: Research shows that women and individuals from underrepresented backgrounds often apply to jobs only if they meet 100% of the qualifications. Reporting to the Program Director, you’ll collaborate closely with clinical staff to assess client needs and ensure client satisfaction. Your primary focus will be client retention, prioritizing efforts to provide each client with a positive treatment experience. You’ll address client concerns by escalating issues to the Clinical or Administrative teams, as needed. By facilitating communication and leveraging available resources, you’ll help management meet clients’ mental and physical health needs, ensuring their ongoing well-being. Additionally, you’ll provide essential case management to support clients during treatment, ensuring their needs are met and that discharge planning is in place for a smooth transition to aftercare services.

Specific essential duties and responsibilities will included, but not be limited to:
  • Developing individualized care plans in collaboration with patients and treatment teams, incorporating after-care planning from the onset of admissions.  
  • Assisting clients in completing surveys at four stages of treatment: admission, in treatment, discharge, and post-discharge.  
  • Facilitating communication among clients, psychiatrists, and other relevant parties.  
  • Coordinating and scheduling third-party appointments for clients, verifying insurance coverage, and referring them to specialized services like substance abuse treatment and vocational training (TMS and Ketamine).  
  • Maintaining accurate and up-to-date case notes and documentation of services provided.  
  • Conducting weekly case management sessions with each client and documenting them in the EMR on the same day.  
  • Completing the discharge case management note within one week following the client's discharge.  
  • Maintaining communication with external facilities, such as substance abuse or eating disorder treatment centers, to ensure seamless transitions and the client’s return to our mental health facility as needed. 
  • Participating in treatment team meetings and case conferences to discuss clients' progress and coordinate care plans with other staff members.  
  • Coordinating and facilitating support groups or discharge planning groups as per the curriculum for clients and their families to address specific mental health issues or coping skills.  
Physical requirements: In this position, you will spend at least 50% of the time sitting and working on a computer. You will lift and carry light objects and parcels and move inside and outside the facility. You must be able to hear and speak English clearly.
